REGISTRATION FORM

 

To register, please fill out the below Registration Form. In order to confirm space, a 200 PLN deposit per camp per child needs to be recevied. This deposit can be paid by cash or card in a GYM Generation gym or by bank transfer on account Nr: 18 1160 2202 0000 0002 6298 4552. ( In the title, please write name of a child / GYM / dates of the camp).

Upon completion of registration, confirmation email will be sent. Registration is not accepted without deposit and registration form.

Payments: We accept bank transfers, payments by cards or cash. Total fee is due at least 7 days prior to the start of the camp.
Registration: Due to limited spaces, registrations are accepted on a first-come first-serve basis. Registration is not accepted without deposit and registration form.
Cancellation & refunds: Cancellation 7 days plus prior to the start date of the camp week is subject to 200 PLN administration fee. Cancellation less than 7 days - No refunds.
Discounts: Family discounts 10% issued for 2nd child, 2nd camp - 5%. Discounts are not cumulative.
Insurance: Parents are required to provide health insurance for the child participating in the camp.
Food: Campers are given a hot lunch and healthy snacks throughout the day. If you would like to pack additional food for them due to dietary requirements, please inform the manager.

GYM Generation reserves the right to photograph and video record all participants involved in GYM Generation programs to be used solely for the purpose of promotional material and publication.
